Referral Process
Anyone who has been diagnosed as having a learning difficulty can refer themselves or can be referred by friends, family, carers or an official body e.g. social worker or community nurse.
To determine the necessary level of support, a well being assessment is required which will be carried out by your social worker. During this assessment you and your social worker will agree how much support you need in different areas of your daily living.
Once a referral has been accepted, an initial visit to the property will be arranged for the service user and their carer(s), allowing both parties the opportunity to assess whether or not the service we offer would be suitable for the needs of the service user. If suitable, a day visit and/or an overnight assessment can be offered.
Once the service user feels comfortable with our service, further overnight stays will be offered and regular respite dates will be discussed with the service user and carer(s).
The referral process may be flexible in order to cater for individual needs.
